3-month Prescription Request Letter

A letter for you to 'fill out' and send to your GP to ask that they check and change as needed, the frequency of your prescription of essential medication, so that you receive them every 3-months.  It also lets them know the importance of approving early repeat prescription requests.

This will allow you to build up a 'stock' of medication for you to use when you are following Sick Day Rules, in line with the NICE Guidelines. 

This letter makes it clear that the request is in line with the recommendations around prescription frequency for essential medications.

This letter has been jointly authored by the ADSHG and Pituitary Foundation,  reviewed by the members of the ADSHG and Pituitary Foundation Clinical Advisory Panels, and endorsed by the Society for Endocrinology.
