Individual Healthcare Plan (IHP) Template
This 14-page template has been designed to capture in one place all the key considerations and essential health information for students with all types of adrenal insufficiency.
This IHP template, developed in collaboration with endocrine clinicians, parents and teachers, is a pivotal document for students with medical conditions in the school environment, bringing together student health information, identified student needs, important resources, and checklists to ensure that everything is covered and reviewed regularly.
The IHP should be completed with input from the young person with adrenal insufficiency, their family, healthcare team, and school representatives.

It includes information from the NICE Guidelines for the Identification and Management of Adrenal Insufficiency, as well as the British Society of Paediatric Endocrinology & Diabetes (BSPED) Adrenal Insufficiency Consensus Guidelines.
